Our Expertise
Personalized and Appropriate Placements
As Physical Therapists (PT) we bring medical and functional expertise that traditional placement agents may lack. We evaluate mobility, strength, balance, and fall risk. We understand complex medical conditions like Parkinson’s, stroke recovery, or orthopedic issues.
Recommend the right level of care (e.g., assisted living vs. memory care) based on functional and health status. Because PTs are trained in rehabilitation and functional assessment, we can match seniors to communities that can accommodate both current and potential future needs.
Advocate for environments that support mobility and physical activity. Ensure the senior has access to proper therapy services if needed.
Holistic Care Coordination
The PT can coordinate with other healthcare providers, families, and care managers to smoothly transition from hospitals or rehab centers to long-term care.
Assist with discharge planning and avoid unnecessary hospital readmissions.
Provide follow-up care recommendations.
